Then choose the box labeled “Good Faith Exam” and select any time, since that appointment is asynchronous (you will not receive a phone call). Please complete the forms thoroughly. Because we provide medical aesthetic treatments, it’s essential that we have a full understanding of your health history to ensure your care is safe, appropriate, and tailored to you. Additionally, you will need to send a copy of your driver's license picture and a makeup-free, non-smiling selfie to hello@beautifullyyou.info. A copy of your driver’s license is the simplest way to verify your identity for legal purposes. These will then be uploaded into our HIPAA-compliant electronic medical record. You will receive reminders for your upcoming appointment—these are to remind you that all required paperwork, including your photos, must be submitted before your selected appointment time.

SERVICES
CONSULTATION
This is a great place to start, we can meet in person or on zoom. During this appointment we can discuss your skin concerns, goals, and treatment options. You will have to schedule another appointment to beging treatment. Consultations are always free!
NEUROTOXIN
Neurotoxins block signals from the nerves to the muscles, which makes that wrinkle relax and soften. The effects are temporary, lasting from 2-4 months.
DERMAL FILLER
Dermal Fillers are gel-like substances that are injected beneath the skin to restore lost volume, smooth lines and soften creases or enhance facial contours. Results last 6-12 months.
PRP HAIR RESTERATION
Platelet-rich plasma (PRP) therapy harnesses the power of your body’s own growth factors to regenerate hair follicles and stimulate new hair growth. Patients own blood is collected with a quick blood draw so it is recommended to hydrate well in advance. Once the blood is drawn, it is placed in a centrifuge (device that spins at high speeds). This action physically separates the solid and liquid parts of the blood: red blood cells (erythrocytes), white blood cells (leukocytes), platelets (thrombocytes), and plasma (liquid). The PRP is then injected directly into the patients scalp. If you’re struggling with hair loss and are ready for a non-invasive solution that provides natural results, PRP is a great option. Recommended every 6 weeks until satisfied with growth, then twice a year for maintenance.
PRP FACIAL INJECTION
Platelet-rich-plasma, or PRP, is a concentrated extraction of platelets suspended in a small amount of plasma. Platelets contain powerful growth factors, nutrients and other bioactive compounds important for tissue repair and regeneration. Your own PRP can be used to soften under eye hollows, pigmentation, improve skin tone, tightness and texture. PRP is an effective nonsurgical way to rejuvenate skin. A series of three is recommended every 6 weeks to see maximum results.
SKIN PEN and MICROCHANNELING
SkinPen and Microchanneling are cosmetic procedures that cause tiny micro-injuries to the skin to stimulate your body's natural healing process and increase collagen production. This treatment is customized specifically for your skin concerns (hyperpigmentation, dehydration, fine lines). SkinPen and Microchanneling come with the option of adding PRP to enhance results.
B12 INJECTION
B12 is an intramuscular injection. Benefits include: an increase in energy levels and concentration, improved metabolism, boosts the immune system, and improves sleep patterns. Injection every 2-4 weeks for optimal benefits.
CHEMICAL PEEL
A chemical peel is a solution applied to remove the top layers of skin. Great for pigmentation, fine lines, sun damage, and scarring. We can discuss your concerns and decide which peel is right for you.
SCULPTRA
Sculptura is the first injectable collagen biostimulator containing microparticles of bicompatible poly-L-lactic acid (PLLA). While hyaluronic acid fillers correct the symptoms of facial aging, Sculptra addresses an underlying cause itself, lost collagen. This service is a journey, it will take 2-3 vials per session with a minimum of 2-3 sessions, scheduled 6 weeks apart. The end game is an improvement in the appearance of skin, wrinkles, facial structure up to two years after the last treatment.
